How do I lose those last stubborn ten to twelve pounds?

I started exercising and changing my diet back in July of 2011, and so far I have losted 45 pounds. The goal was to lose up to at least 55 to 60, and I do understand that the more weight you lose the more harder it would be to take it off. I do exercise everyday with cardio being every Monday, Wednesday, and Friday 2 to 4 hours while strength training is every Tuesdays, Thursdays, and sometimes Saturdays for 45 mins to an hour. The funny thing about this is that Im not getting tired easily and Im still not losing that extra wieght. Whats going on and can you help me? Thanks.
